Clady Medical Centre is part of the HSE Chronic Disease Management (CDM) Programme. This programme is free for patients who have a medical card or GP visit card and have a diagnosis of one or more of the listed conditions below.
- Type 2 diabetes
- Asthma
- Chronic obstructive pulmonary disease (COPD)
- Cardiovascular disease, including heart failure, heart attack (angina), stroke and irregular heartbeat (atrial fibrillation)
It involves 6 monthly check-ups relevant to the particular chronic illness. The first visit is to the nurse who will perform measurements and appropriate blood tests. A follow up visit is arranged with the doctor with a review of the results and formation of a plan for the next 6 month. Clady Medical Centre is happy to provide this service to non-medical card patients also. Please discuss this with your GP who can outline the programme and the costs of this service.

- Peak Expiratory flow rate measurements (PEFR); a simple breathing test which measures airflow from the lungs & can be used to both monitor & help to diagnose asthma.
- Nebuliser treatment for acute asthma attacks. Medication can be delivered by inhaling the relieving drug in the droplet form created by a nebuliser.
- Smoking Cessation: This is a difficult task and we advise you get as much help as possible.
- Inhaler technique: how to use your inhaler correctly. As time passes, inhaler requirements may change and technique needs to be reviewed regularly
- Referal to a Respiratory Physician in severe cases
